顔面神経麻痺は完治との診断が出ました

1カ月ぶりの耳鼻咽喉科診察

先生と向き合うと、マスクを外して

先生が先導して、ウインクから様々な表情をしていく

一通り終えると、「完治しましたね」とのお言葉

処方する薬もなく、手ぶらで帰るのは珍しい

思い返すと、発症は5月の連休中だったが、

すぐに休日急患に飛び込んで、

7日間で合計500mlを超えるステロイド点滴を受けれたこと

点滴を終えた1週間後から

ペインクリニックで星状神経節ブロックをはじめた

首への注射は恐怖感があったが、

眼や口の動きにすぐに効果が出たので

回数は13回に及んだ

幸運だったのは、

このペインクリニックで

コロナウィルスワクチン注射が打てることになり、

耳鼻咽喉科で完治の診察を得たその日の夕方に、

自治体で打つより2週間早く接種することができた

関節リウマチでの抗がん剤投薬により、

基礎疾患での登録だったが、

周りの同様の知人のケースを聞いていると、

基礎疾患のかかりつけ医による接種の機会提供が

最も早いタイミングの接種となっているように感じる

基礎疾患の場合、

自治体がそれを把握することは難しく、

症状をよく知るかかりつけ医が接種に関与するのは

とても自然なことと思う

とはいえ、関節リウマチや耳鼻咽喉科でかかっている総合病院では

ワクチン接種の話題はなく、

たまたま顔面神経麻痺でお世話になることになったペインクリニックの先生が

接種に関わってくれたのは、不思議な縁のあやとなった

さらに言えば、ワクチン接種1回目の日に顔面神経麻痺が完治したことになり、

ペインクリニックでの星状神経節ブロックも不要となったので

先生には申し訳ない気持ちになるが

ありがたく2回目の接種もお願いしている

やはり何度も注射していただいた先生によるワクチン注射は

安心して受けられる

ペインクリニックを推してくれた

知人にお礼の品を受け取ってもらったのは

The original is Jean Poiret’s stage play “Mr. Lady Mr. Madam”,

which received the Tony Award in 1984.

Like my favorite Gay’s movie “Torch Song Trilogy”

It ’s painful, but it ’s heartwarming.

 

On the other hand, this “La Cage au Fall” is

a play style that makes you laugh all the time.

Probably this is the reason for receiving it in Japan

As proof of that, as the role of Zaza

Known as Masachika Ichimura in Japan,

Feelings for the song

This is the French musical based on Victor Hugo’s novel “Notre-Dame de Paris”.

Disney’s “The Hunchback of Notre Dame” is the same as the original.

The French musical has a much more original love triangle,

drawn with a muddy touch.

While remembering the movement of Quasimodo’s body by the composition of the song

Eventually, I was dragged into a dark and deep pool and couldn’t escape.

Feelings for the song

I saw this movie on DVD many times,

Which received Cannes Film Festival Grand Prix.

 

Michel Legrand’s beautiful melody is my favorite.

Since Jazz sounds are woven from time to time in this movie,

It ’s a miracle that Danielle Licari could dub Catherine Deneuve’s song.

It is not necessary to look at the image.

I can be satisfied by just hearing the sound in the room,

especially in Rainy days.

 

Regrettably he passed away in January 26th 2019.

However his passionate art will live in my soul eternally.

 

The theme of director and screenwriter Jacques Demy is also poetic.

The points of theme are

ロジャース&ハマースタインの『南太平洋』には多くの名曲がある

その中でも代表的なSome Enchanted Eveningは

特にUSAの多くのシンガーに良く歌われるポピュラーソングである

 

自身にとってこの曲を奏でる代表者は、マントヴァー二・オーケストラだ

城達也のジェットストリームを聞きながら、

出張帰りにうたたねを打ちながら流れてくるこの演奏は

横たわる体が浮いていくような解放感がある

 

シンガーで、ぜひお薦めしたいのは、

日本語詞である「魅惑の宵」を歌った弘田三枝子だ

この人が歌うと

もはや『南太平洋』の世界を離れ、

昭和のクラブラウンジで、ブランデーなど傾けながら

聴くジャジーなインドアの世界に連れて行ってくれる

口腔内の細菌をロイテリ菌で減らす

一難去って、また一難

耳周辺の痛みに悩まされた顔面神経麻痺も、

星状神経節ブロックによってようやく落ち着いてきた

ところが、一難去って、また一難

予定していた人工歯根に乗せる歯も手を付けることができるかと思っていたが、

いざ型を作ろうとして診てもらうと、

しばらくの間、人工歯根の上に義歯を乗せて様子を見ることになった

 

歯茎が安定しない原因

理由は、

人工歯根周りの歯茎・歯肉がなかなかしっかりとした硬さにもどらず、

出血している状況に変化が見られないからである

人工歯根の手術をしてすでに10カ月

その間、一日3回の歯磨きや、就寝前のイソジンなどでの人口歯根周りの消毒など、

丹念にまた慎重に続けてきたが、改善がほとんど見られない

一般的には、このような歯肉の炎症や腫れは、歯周病変と呼ばれ

歯周病原因菌による細菌感染によるものとされている

そしてその原因は、

関節リウマチで3年間服用している免疫抑制剤であるメトトレキサートが有力だ

そのうえに、ここ1か月間、顔面神経麻痺の治療のため、

540mgものステロイド剤を注入したわけで、

細菌にはきわめて抵抗力のない状態となり重症化したことは容易に想像できる

さらにメトトレキサートの副作用として骨粗鬆症が進行すれば、

歯周組織での骨形成能が低下して、

状態を悪化させている可能性がある

 

求められる対策

関節リウマチにかかって3年

その治療薬である免疫抑制剤によって

ウイルスへの抵抗力が落ちてきて

顔面神経麻痺、歯茎の炎症と

次々と新たなトラブルがやってくる

この悪循環をどう断ち切るか

もっとも望ましいのは、

メトトレキサートを減らすことだが

しばらくはDo Check Do Check

一気に減らすことは難しい

 

すぐにとることにした予防策

1.ロイテリ菌

なかなか治らないことをツイートしたとき、

活性酸素を取り除き炎症を抑制できるとされているが、今回は見送り
